Custom Foot Orthotics includes: Consultation and examination, visual inspection, static and dynamic gait scan and analysis, Paris plaster casting-molding or 3-D laser imaging scan casting, lab fees, a pair of custom foot orthotics, fitting and dispensing, and a follow up visit. $135 is for consultation and examination, visual inspection, static and dynamic gait scan and analysis. $360 is for Paris plaster casting or 3D volumetric digital casting, lab fees, a pair of custom foot orthotics, fitting and dispensing, and a follow up visit.
Our office will provide you with all the necessary supporting documents which you need to submit to your extended benefit / insurance provider. In some cases, you may need to get a referral from a Medical Doctor which needs to be submitted to your extended benefits.
